A memorandum at the end of an affidavit stating where and when the affidavit was sworn, followed by the signature and description of the person before whom it was sworn.

A medical examination after death
Hint: Post = After. 'Mortem' is like 'mortal', which means 'Subject to Death'. Hence, 'After Death'

Without appointing a day; indefinitely.

Under penalty. A writ commanding a person's attendance in court of justice. Generally this is a witness summons.
